Gender Dysphoria (GD) is associated with several psychiatric conditions, but the causal links are not known. We note that some of these conditions are in turn associated with physiological masculinization, and explore the relationship between GD and a number of robust correlates of testosterone (Autism Spectrum Disorder, Left-handedness, 2D:4D Ratio, being male, and male heterosexuality). Individuals with GD exhibit elevated scores in the direction of high testosterone and masculinity. It is further shown that other identity disorders (or dysphoriae) are associated with the same correlates of elevated testosterone, such as autism. Consistent with the Extreme Male Brain Theory of Autism, we argue that GD is likely a reflection of Autism Spectrum Disorder traits indirectly leading to anxiety and a weak and inaccurate sense of self. We tentatively suggest that Blanchard’s Transsexualism Typology most simply theoretically explains our findings.
